For 2024, office visit codes 99202 through 99205 and 99212 through 99215 have been revised to remove the time range in minutes from each code.
This revision now indicates the minimum time that must be met or exceeded.
When using time to choose E&M services, document the specific time in the notes; and if the code choice is by medical decision-making, it does not need to meet the minimum time.
Most carriers prefer and want time documented in ‘from and to’ time.
